Transcripts are free to Sandy High School students. The fee for graduates is $2. To request a transcript, please fill out this form.

The following information will be needed: Student ID#, full name of student, graduation year, date of birth, number of transcripts needed (official and unofficial), mailing address and the contact name and phone number of person requesting the transcrips.

It is also the time to order your final, 8^th Semester Transcript. If you are going to college you must have a final, official transcript sent from Sandy High to your college. The counseling center will be closed in July and part of August so Do Not Delay or forget to order your final, 8^th semester transcript today!  This final transcript, showing you graduated, will be sent by the end of June.
